In safety-critical industries, common sense without rules knowledge leads to dangerous improvisation. Rules encode hard-won lessons from previous accidents — they exist precisely because common sense alone is insufficient.

Myth

"Safety rules are important, but experienced operators know when a rule can be safely adapted to the specific situation."

Truth

The situations where experienced operators feel most confident adapting rules are often those where the rule exists precisely because similar judgements previously produced accidents. Safety-critical rules encode the lessons of failures that the individual has not personally experienced — which is why they feel like bureaucracy until the accident they were designed to prevent occurs.
