Prescribing for an individual patient or animal requires integration of the diagnosis, the drug's pharmacology, the patient's comorbidities and current medications, the patient's organ function (renal and hepatic clearance), and the risk-benefit balance for that specific individual at that point in time. The recommended dose is a starting point for a population average — expert prescribers adjust it for the individual. Prescribing errors are among the most common and consequential clinical errors, which reflects the genuine complexity of translating pharmacological knowledge into safe individual prescriptions.

AI clinical decision support tools are providing real-time prescribing alerts at the point of care — flagging interactions, dose range violations, and renal adjustment requirements that prescribers may overlook. - 2.Practise calculating doses for drugs with weight-based dosing — antibiotic doses, insulin doses, chemotherapy doses — until the calculations are reliable under time pressure.
- 3.Review an adverse drug reaction case — identify what the prescriber should have checked before prescribing and at what point in the prescribing process the error could have been caught.
- 4.Complete a drug prescribing OSCE (Objective Structured Clinical Examination) station — practise prescribing for common presentations, checking interactions, and writing a complete, legally valid prescription.
