"Triage means treating the most injured person first."

Truth

Mass casualty triage is explicitly NOT about treating the most injured first — it is about treating the most salvageable first. A patient with catastrophic injuries requiring hours of surgery to achieve uncertain survival may be categorised as Expectant while less severely injured casualties with good survival prospects receive immediate intervention. This is a psychologically and ethically demanding departure from normal clinical practice, which is why training and mental preparation are essential components of major incident response.

Research & Outlook

The UK's Major Trauma Networks and NHS England Major Incident Response framework have significantly improved coordination between pre-hospital responders and major trauma centres, reducing the time from triage to definitive care. Wearable physiological monitoring devices and digital triage platforms (replacing paper tags with electronic triage tools with GPS location and live dashboard updates) are being trialled in NHS major incident exercises. Mass casualty events remain unpredictable, and the fundamental skill of making rapid, accurate triage decisions under extreme stress without technological support remains essential.
