Protocol adherence is the minimum requirement. Processing quality also depends on fixation adequacy before processing begins, dehydration and clearing completeness, wax infiltration temperature and time, embedding orientation accuracy, section thickness consistency, and staining protocol execution. Each step can introduce artefact that compromises diagnostic quality, and the consequences of each error are different — requiring the practitioner to recognise what went wrong and where.
Research & Outlook
Histopathology processing is being transformed by digital pathology — the scanning of glass slides to create high-resolution whole slide images (WSIs) that pathologists report on screen rather than at the microscope. This requires histopathology laboratories to achieve consistently high slide quality standards (section thickness, staining intensity, absence of artefacts) because digital scanning amplifies imperfections that might be tolerated in glass slide review. AI-powered image analysis tools for detecting cancer cells, grading tumours, and quantifying biomarker expression are being integrated into digital pathology workflows, but their performance is critically dependent on the quality of the underlying slide preparation. Automated grossing systems that use robotics and 3D imaging to systematically sample resection specimens are in development, with the potential to standardise and document grossing decisions that are currently undocumented. Spatial transcriptomics and proteomics platforms that extract molecular data from histological tissue sections are creating new roles for histopathology laboratories in molecular pathology workflows.

Independently grosses a wide range of surgical resection specimens to RCPath minimum dataset standards, operates automated processing and staining platforms, troubleshoots tissue processing and staining artefacts, and performs immunohistochemistry staining for diagnostic markers. Trains junior staff and contributes to departmental quality assurance.
Grosses complex oncological resections (total mesorectal excision, Whipple's procedure, radical prostatectomy), validates new staining protocols and immunohistochemistry antibodies, manages laboratory accreditation (UKAS ISO 15189) for histopathology, and leads implementation of digital pathology workflows. Contributes to national proficiency testing scheme design and RCPath guideline development.
How to Practise
- 1.The pathway to histopathology laboratory practice in the UK is through an IBMS-accredited BSc Biomedical Science degree followed by an NHS training post leading to HCPC registration as a Biomedical Scientist — there is no independent route to clinical histopathology competence.
- 2.Study histological anatomy and tissue morphology systematically — understanding normal tissue histology is the prerequisite for recognising diagnostic features and embedding tissue specimens in the correct orientation for the pathologist.
- 3.Complete the IBMS Certificate of Competence portfolio for histopathology, documenting assessed competences in specimen reception, grossing, processing, embedding, microtomy, and routine staining under the supervision of a registered BMS.
- 4.Study the Royal College of Pathologists (RCPath) minimum datasets for cancer resection specimens — these define the grossing and sampling requirements for each tumour site and are the national standard against which histopathology laboratories are audited.
