"If the computer doesn't flag it, it's safe."

Truth

Electronic interaction checkers are sensitive but not always specific. Many flagged interactions are clinically insignificant, and some unlisted combinations carry real risk. Professional judgement is always required.

Myth

"Drug interaction checking is mainly about identifying contraindicated combinations — if the combination is not flagged as contraindicated, it is safe to prescribe."

Truth

Contraindication flags identify the most severe interactions but miss clinically significant interactions that fall below that threshold. Interactions that alter drug levels, reduce efficacy, or increase adverse effect risk without meeting the contraindication threshold require practitioner awareness and judgement that software flags alone do not provide.
